Total Joint Replacement : An Eastern India Scenario

Dr. S. Dasgupta, MBBS (Ca/)FRCSE,FRCS

Joint Replacement was slow to take off in this region -Why?

Jomt Replacement Surgery is a major procedure, improving the quality of life. However, it is not a life saving procedure, like a cardiac bypass surgery, so patients experience hesitancy in taking quick decisions. Further, teaching hospitals in Kolkota responded to the need for joint replacement facilities relatively late, by which time centres in Mumbai and Chennai emerged as centres of choice, well recognized by patient population. These factors have resulted in fewer numbers of hip and knee replacement surgeries performed in Kolkata, even though a substantial number of Eastern region residents require this treatment .

Present Scenario

Hip replacement surgery program was started in the East for the first time in "CMRI". More than 30 years ago. Dr. S. Bhattacharjee was the first surgeon to do this procedure at CMRI. Since then, we have been performing hip replacements on a regular basis, with !=1ood results.

We normally do 'Total Hip Replacement' using all cemented prostheses because of the price factor. Non- cemented types are available in the market; though these are extensively used abroad, they are extremely expensive and this limits their usage in India. Hybrid implants with a press fit ace tabular cup and cemented femoral component are also gaining popularity. The type of instrumentation required varies with the type of prosthesis used, but the basic cuts/approaches remain the same. Patients have a choice of implant. Expert guidance is given to their clinical/functional needs in keeping with their budget.

Knee replacement surgery program was also started first in CMRI, in the mid 80s, again by Dr. S. Bhattacharjee, in conjunction with Dr. Dilip Pal of Texas, USA. Subsequently this procedure has been done on a regular basis at CMRI by me and my colleaques.

  • Types of operatior implants of Knees

  • Total condylar/patellar replacement semi (all cemented)

  • HOP Tibi01 Component Met01 bock or 011 Poly

  • Patella all poly (no metal back)

Projections for the future

There is a spiraling need for joint replacement surgery, due to an ageing population. CMRI has the expertise and excellent facilities to provide this service. To these we also add judicious patient education and motivation, to provide the best dividends for patient care.

We also offer packages to bring the cost within reach for the majority of our patients.
